By text with a short link, by email, on the counter tablet, or from a printed code. Same share panel as Skin surveys and Free Tans.

The template for that session type, with your questions, on their phone. A date question, a signature, done.

The app files it
A PDF with the record id in its footer, on the customer's file, under the salon. The booking sheet shows Signed.

UV consent for beds, spray consent for the booth. A customer signs the one their booking needs.

Sent, not signed
A chip on Waivers, on Customers and at the till, so nothing is missed at the bed.
No waiver, no session
A session that needs a waiver does not start until it is signed. Letting one through takes a manager's passcode, and a report lists each one.
Minimum age
Set a minimum tanning age from 16 to 21. It is checked at booking and at the till, and no passcode gets past it.
